i) Application form for passport for children born in South Africa can be filled up online at http://passport.gov.in/nri.
ii) Self-attested copies of the parents passports first two pages and last two pages
iii) Copy of passport page containing visa/work permit/permanent residence status of both the parents
iv) Three passport sized photographs (Coloured Photos of 5cm x 5cm with white background)
v) Proof of Residential address in South Africa
vi) Both parents should sign on the form at the prescribed column under declaration by parents/guardians in case of a minor. In case of infants thumb impression should be affixed within the box on Page No. 1 and Page No. 2 after Serial No. 26 of the form.
vii) Birth registration at the Consulate and birth certificate issued by South African Home Department and local hospital in case of South African born babies.
viii) Fee of Rand 825/- (including ICWF Fee of Rand 25.00) is charged for the service. Payment can be made by EFT/Credit/Debit Card. Details of the same are available on website www.blsindia-southafrica.com. Please note original proof of payment is required and photocopy, faxed copy or scanned copy is not acceptable.
ix) Five-years validity passport will be issued
x) Original passport/documents will be returned immediately after verification
xi) Any other document as specified by the Consular Officer;
